Medical Records


All medical records are strictly confidential.  Information from records will NOT be released to anyone without written authorization of the patient, except as required by law.  Student Health & Counseling Services Medical Records can assist you with the following services.

  • Requesting medical records for yourself
  • Clearing Immunization Holds
  • Sending medical records from Student Health & Counseliing Services to another health facility or provider

Confidential

Complete medical records are maintained on all Student Health & Counseling Services patients. Information in student medical records is confidential, and access is restricted to authorized SHCS personnel. We will not notify parents, professors or anyone else regarding your health care, except in a life threatening situation or if you are a minor. It is important that you notify your provider if your parent plans to call and it is acceptable to you for us to discuss your case with them. Your medical records are strictly confidential and are not available to anyone unless you provide written consent or when required by public health law, by court order or when subpoenaed.  Retention of medical records, electronic or paper, will be retained for a period of 7 years after the last date of service.

Copies of Medical Records

Patients who wish to obtain a copy of their records can contact the Medical Records department at 916-278-6461. If you wish to release your medical record to anyone, you must first sign a release form, which is available at the Student Health Center, or you can download one here.

Records may take up to two weeks for processing. Records requested will incur a $15.00 fee.
